1. What do I need to bring to my first visit ?
- All insurance and prescription cards. It is also suggested that you bring a list of all prescribed and over the counter vitamins or supplements that you are currently taking.
- Bring copies of medical records or test results or ensure that your physicians send these to our office at lease one week prior to your scheduled appointment.

- Bring a referral from your health plan if one is required, or ensure that authorization has been obtained by your primary care physician, if one is required.

- With Pre-Authorization we can treat most industrial patients, this includes State Comp Fund, Gallagher Bassett, U.S. Dept of Labor, etc.
- Lifewise
- Medicare (and many Senior Medicare Replacement Plans)
- Multiplan Network
- MedPay
- With verification of eligibility and assignment of benefits, we caan treat most MedPay Patients. We do not, however, accept liens.

3. What are the office hours ?
- Monday through Thursday, 8:30am - 5:00pm (this schedule may occasionally vary for holidays)
- Friday, 8:30am - 4:30pm
- After Hours: Our providers are available by phone 24 hours a day for emergencies.
- Please UNBLOCK your phone line if awaiting a return call from our office.
4. How long does it take to get a prescription refilled ?
- Please allow 2 business days for refills of routine medications. Out of formulary medications may require up to 5 business days depending on the insurance carrier approval requirement.
