We hope the information you find here will help you become familiar with our Physicians and the type of service we offer. We try to provide the best service available to our patients. You will find copies of our office forms available for download to be filled out at your convenience prior to your first visit of for our established patients to update your current address, phone or insurance information and be brought in with you at your next scheduled appointment.
If you are new to our practice please read our Office Policy and Procedure Sheet. This information is designed to help you understand that your health care with our practice is very important to us and we want to make sure that you, our patient, understands that there are procedures that must be followed to provide you with quality care in a timely basis. It requires that both parties understand that our staff is doing everything possible to answer your questions and ensure you our patients are taken care of.
We do respect your privacy and will keep your information confidential. Please read the Uses and Disclosures of Protected Health Information, Notice of Privacy Practices that our office uses to describe how medical information about you may be used and disclosed and how you can get access to this information. We also have a Request For Confidential Communication that we require you to fill out so that we may have in our records, the manner in which you wish to have the staff at Four Peaks Neurology, P.C. contact you with sensitive medical information.
As with all medical practice it is sometimes necessary to share medical records with other physicians you may be seeing in addition to other service that may require your medical records. We therefore, have a Medical Records Release Form that we require you to sign that will designate the party you wish these records to be released to.
