Click the following link and you will have access to a printable PHYSICAL FORM.  It is essential that doctors fill out the physical form required by the OHSAA.  Any other physical form will not be accepted.  An athlete MUST have the form in prior to participation.

NATIONAL CLEARING HOUSE

The NCAA Initial Eligibility Clearinghouse is the body assigned by the NCAA to certify student athletes for athletic eligibility based on academics in their first year of competition at Division I, Division I-AA, and Division II institutions.  The Clearninghouse collects transcripts and test scores and them makes a ruling on your academic status.  Click below for details:
