This deposit is nonrefundable unless the program is cancelled by The Tandana Foundation, in which case it will be refunded in full. The remainder of the payment will be due 6 weeks before the program start date.
PHOTO RELEASE: The Tandana Foundation (TTF), or individuals or entities authorized by TTF may photograph and/or videotape program activities. Unless we receive your objection in writing before the start of the program, you authorize TTF to use your photo, video image, or quotations from correspondence or evaluation forms for sale or reproduction, in any manner TTF desires, for advertising, display, audiovisual or other use.
NON-DISCRIMINATION: TTF does not discriminate in its respective employment or admissions policies on the basis of race, color, gender, religion, national or ethnic origin, age or disability.
TTF is not responsible for the costs incurred by Participants due to TTF's changing the date or cancellation of any program. TTF strongly encourages Participants to purchase trip insurance, in the event of program changes or withdrawal from the program after full payment has been made. Participant acknowledges that if he/she is dismissed or departs early from a program, for any reason, no refunds can be granted. Participant is responsible for all costs of early departure whether for medical reasons, dismissal, personal emergencies or otherwise.
TTF reserves the right to dismiss any participant from the program that staff believe, in their discretion, presents a safety concern or medical risk, is disruptive, or otherwise conducts him/herself in a manner detrimental to the program.

I have accurately completed the Application and have read, understand and agree to the Terms of Agreement outlined here. All aspects of my relationship with TTF will be governed by Ohio law, and any mediation, suit, or other dispute with TTF must be filed or entered into only in Ohio.
